Good afternoon!

Spring sports is right around the corner! If your child/children are interested in any spring sports for this school year please complete all steps below in order for your child to start official practice which will be February 23rd, times will be communicated from coaches.

 To complete your child's sports registration:

1. Returning or new student registration must be completed each school year using Infinite Campus. (Already Completed if your student is enrolled currently).
2. Complete this sports registration form and submit it.
3. Email or drop off your child's Physical Form signed by a doctor to jordanmadsen@re3j.com.  Physicals are good for 365 days, please make sure your child has a current physical.
4. Email or drop off proof of insurance to jordanmadsen@re3j.com.
5. Payment of High School Sports fee  $100.00 per sport with a cap of $200.00  (Payment made using Infinite campus parent portal or drop off a check to the high school attention Jordan Madsen). Per handbook, all sport fees must be paid before the first game/meet. If you are unable to pay, you will be asked to set up a payment plan.
